We implement all Continuum of Care Permanent Supportive Housing Programs in the mid-shore, which consists of 63 units of housing for families and individuals who are experiencing homelessness and have mental health disabilities. Program participants locate housing units throughout the five county region that are safe and habitable, a percentage of the participant’s income goes towards their housing cost and the program pays the remainder in a subsidy to the landlord. Participants are connected with case management services and work on personal goals to achieve self-sufficiency.
